Athlete Medical Report Form
This form is to be completed by all athletes that have nominated for, or intend to nominate for, National Selection in cases where they have had their training affected for more than 24 hours due to either injury or illness.
It is the athlete’s responsibility to ensure that their treating doctor or physiotherapist complete this form at their initial consultation. The form may be downloaded, or printed off, and taken with the athlete to their consultation.
National Squad Sports Science/Medicine Reconciliation Claim Form
As part of the National Training Squad process, nominated athletes who train in centres other than the AIS are able to claim for some of the costs incurred for recognised sports science or medicine costs on a cost recovery basis. Read the associated policy here.
